To help meet your goals your care team will recommend appropriate guidance to ensure you or your loved one is safe and secure in your home, at an Assisted Living Facility, Independent Living Facility, Group Home or Nursing Home.

Our Comprehensive Assessment will identify Medical, Psychological, Safety, Legal and Financial needs. We can also help identify eligibility for entitlement programs such as Medicare Part D, VA benefits and Medicaid benefits. Our Case Management Team will coordinate scheduling of appointments, communicating with physicians and providing assistance with Community Resources.

We also offer our Crisis Intervention Team, which is a team of specialist who works around-the-clock to ensure all your healthcare needs are met such as needing immediate Hospital or Urgent Care attention. Our Crisis Intervention Team will coordinate all communications with your family, physicians, and hospital staff. We will advocate for you ensuring your number one interest is in mind, which is what is best for you.

Our goal is to keep you independent in your home as long as you wish, but if ever a need arise to downsize, we can assist you with preparing your home to place it on the market for sale or by conducting an estate sale for you and finding appropriate housing for you using our placement services. We will provide several options to you from retirement communities to long-term care communities. We will coordinate all moving activities for you from packing to unpacking with a professional moving company and organizer.

We will coordinate any and all of your house repairs or house updates like placing safety equipment in your home such as bathroom grab bars, wheel chair ramps and door widening for wheelchair access. Our team of housekeepers will ensure your house is tidy and well-organized. Our maintenance team will ensure your landscape is maintained and your general house condition is kept up-to–date such as needing a paint touch up.

Our team of Caregivers will ensure your personal needs are met such as bathing, eating, dressing, and toileting. Our team of Caregivers will also cook, clean, take care of your laundry and shopping needs and escort or transport you to your medical or social appointments or engagements.
